I've put some basic form fields stuff at:

My form building skills are a bit limited really, so I'd appreciate feedback on
the code and what needs doing to it to make it work! Am I at least getting the
basic structure right?

Chris - you mentioned stuff about accessibility with forms - would it be easier
if we got together and went through this? There's a number of other things I'd
like to go through as well!
