Yeah, looks fine, I have added a couple of fieldsets and legends and
I have set up a label for the first field -- test it, click on the
label and the input field should get focus, copy this for the other
elements.

Could do, or we could do it on irc, I'll not have time to set the
form handler up today (have to leave early to do child care) but
I'll get on it tomorrow.
